How did the equestrian sport of Show Jumping begin and develop? (overall research question)

How did the equestrian sport of Show Jumping begin and develop?

To answer this overall question I will try to find out:
     1.  How the main branches of show-jumping (hunter/jumper, eventing, stadium jumping) evolved. Focus mainly on the development of the hunter and equitation
     2.  How did show-jumping become a competition sport?
     3.  How did fox hunting influence modern-day jumping sports? History of the fox hunt.
     4.  Did show-jumping originate in Europe? If so, how did it become popular in america?
     5.  Evolution of competition rules and modern day jumping equipment (saddles & bridles)

What I already know about the topic:
  • Show jumping evolved from the horseback fox hunt.
  • Jumping is one of the most popular equestrian sports.
  • There are different forms of show jumping, such as hunter and equitation (judged) and jumpers/stadium jumping (timed or clear round based)
  • Show Jumping didn't originate in the US (not sure where exactly)
